Findings
The facility failed to provide a safe environment for Alleged Victim (AV) by failing to provide basic care or services necessary to maintain the health and safety of AV which resulted in AV receiving an unexplained bruise on his/her right arm. The allegation that Alleged Perpetrator 2 (AP2) physically abused AV was investigated and findings were inconclusive. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
