Findings
The facility failed to provide appropriate services to the Alleged Victim (AV) including assistance with eating, transfers and maintaining skin integrity. The failure resulted in the AV experiencing lack of basic care and services, and a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
