Findings
The facility failed to follow the Alleged Victim's (AV) care plan to have his/her bed alarm in place to reduce risk of fall or injury. The failure resulted in AV experiencing an unwitnessed fall causing skin injury and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
