Findings
The facility failed to ensure supervision and staff support regarding known fall risks related to the Alleged Victim (AV). On or about May 5, 2020, AV suffered a non-injury fall, however, because there was no staff available in the area of his/her room, AV spent an unknown amount of time on the floor, placing him/her at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
