Findings
On or about May 14 2020, Alleged Perpetrator 2 (AP2) hit the Alleged Victim (AV) in the face, causing swelling to his/her lip. AP2’s actions are considered physical abuse, which is a violation of resident rights and constitutes abuse. The facility failed to protect the resident from physical abuse which violates Oregon Administrative Rules.
