Findings
The facility failed to follow the Alleged Victim's (AV) care plan to ensure he/she does not leave the facility unsupervised. The failure resulted in AV being unsupervised outside of the facility for approximately 30 minutes, causing risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
