Findings
On or about May 17, 2020, Alleged Perpetrator 2 (AP2) was rough with the Alleged Victim (AV) while trying to dress him/her for bed. AV was being resistant to care and AP2 grabbed AV's arm to place it into his/her nightgown. On or about May 19, 2020, AV was found to have a bruise on his/her arm. AP2 is responsible for physical abuse. The facility failed to protect AV from physical abuse and failed to adequately train staff to deal with residents with behavioral disturbance and the facility's failure's are a violation of resident rights, are considered neglect of care and constitute abuse.
