- The facility failed to provide a safe environment for Alleged Victim (AV), which resulted in AV leaving the facility unsupervised. On or about May 27, 2020, AV was found disoriented on the driveway of a home approximately 0.2 miles away from the facility. AV was stuck in a squatting position with his/her pants around his/her ankles and had toileted on the driveway. After being returned to the facility by law enforcement, AV left the facility on the same night without alerting staff and AV was found on a bench outside the facility. An investigation determined that AV had a history of toileting in public areas, had a history of falls, and staff were aware of safety concerns associated with AV leaving the facility unsupervised. The facility's failed to implement preventions/interventions to ensure AV's safety which is a violation of resident rights, is considered neglect of care and constitutes abuse.
