Findings
The facility failed to implement interventions and appropriately care plan related to the Alleged Victim's (AV) behaviors and times of refusing care. The failure resulted in AV wearing the same clothes for multiple days and being found soaked in urine, causing unreasonable discomfort and a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
