Findings
The facility and the Alleged Perpetrator 2 (AP2) failed to provide appropriate services and supervision related to the Alleged Victim's (AV) history of falls. The failure resulted in AV experiencing an unwitnessed fall sustaining a hip fracture, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
