Findings
The facility failed to provide a service for Alleged Victim (AV) to maintain his/her health and safety which resulted in unreasonable discomfort and loss of dignity. Due to AV's motorized wheelchair not operating safely, AV had to use his/her manual wheelchair for about three weeks which limited AV's ability to move throughout the facility and also caused AV additional back pain. The facility's failure to properly maintain AV's motorized wheelchair is a violation of resident rights, is considered neglect of care and constitutes abuse.
