Findings
The facility failed to provide a safe environment for the Alleged Victim (AV). On or about May 29, 2020, AV had an unwitnessed fall resulting in a bruise to his/her face. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
