Findings
The Alleged Victim (AV) was a known exit seeker and elopement risk. On or about June 6, 2020, AV exited the facility without assistance. According to documentation, there was no injury to him/her. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
