Findings: The facility failed to provide a safe environment for Alleged Victim (AV) which resulted in AV falling in the common area when AV was left unattended. On or about June 3, 2020, AV was seated at the dining room table unattended. Witness 1 (W1) attempted to assist AV to a standing position which resulted in AV falling to the floor. Due to AV's complaints of pain, AV was sent to the hospital for treatment and was diagnosed with a fracture and pelvis injury. The facility failed to provide the basic care or services necessary to maintain AV's health and safety, which resulted in physical harm to AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
