Findings
The facility failed to assess and intervene according to the Alleged Victim's (AV) change of condition and increased confusion. The failure resulted in AV being found approximately 1.4 miles away from the facility lost and confused causing risk of serious harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
