Findings
Witness 1 (W1) has a known history of being protective of his/her room and the area near his/her room and Alleged Victim (AV) has a known history of wandering throughout the facility. On or about June 9, 2020, AV was physically attached by W1, when AV wandered into W1's hallway. Staff were instructed to redirect AV from wandering in or around W1's area. On or about June 15, 2020, AV was found on the floor in front of W1's door. A review of video footage revealed that W1 had pushed AV when AV was too close to W1's room. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
