Findings
Alleged Victim (AV) had a history of falls, had a change of condition and his/her care plan was updated to include one person assist with ambulation. On or about June 11, 2020, AV was observed walking alone, tripped over his/her feet and fell. AV sustained a laceration and a facial fracture. The facility failed to follow AV's care plan which is a violation of resident rights, is neglect of care and constitutes abuse.
