- On or about June 16, 2020, the Alleged Victim (AV) was sent to the hospital with a significant change of condition. It was reported that AV had a stroke, however, on June 20, 2020, the hospital determined that AV had a fractured hip from a fall that occurred on June 13, 2020 that was never documented by facility staff, therefore, AV was not assessed immediately after the fall, leaving AV in pain for days from the fractured hip that required surgery. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
