- The facility failed to provide a safe environment for Alleged Victim (AV) which resulted in an altercation between Witness 1 (W1) and Alleged Victim (AV). On or about June 16, 2020, W1 entered AV's room and had an altercation with AV resulting in AV sustaining a skin tear on his/her arm. An investigation determined that W1 has a history of aggressive behaviors and had injured another resident on or about June 10, 2020. The investigation determined there were no documented interventions implemented between June 10, 2020 and the day of the incident on June 17, 2020. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
