Findings
Alleged Victim (AV) and Witness 2 (W2) had an altercation that caused injury to AV. W2 is care planned with aggressive behaviors and frequent agitation. The facility failed to provide a safe environment and have enough staff available to redirect W2, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
