Findings
On or about June 23, 2020, Alleged Perpetrator #2 (AP2) failed to follow the Alleged Victim's (AV) care plan to transfer him/her using two people. During AP2's transfer of AV, he/she fell and received an injury. AP2's actions are considered neglect of care and constitute abuse. The facility's failure is a violation of Oregon Administrative Rules.
