The facility failed to provide appropriate services to maintain the health and safety of the Alleged Victim (AV). The failure resulted in AV failing to receive services according to his/her needs resulting in unreasonable discomfort, and loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
