Findings
The facility failed to follow the Alleged Victim's (AV) care plan to perform hourly checks to prevent falls and/or injury due to his/her fall history. The failure resulted in AV falling from him/her bed causing a skin injury and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
