Findings
Alleged Victim (AV) is care planned for showers twice a week on Sundays and Thursdays. On or about June 25, 2020, AV did not receive his/her shower causing AV to feel as though she/he smelled for not being showered for six days. The facility failed to follow AV's care plan causing AV unreasonable discomfort, which is a violation of resident rights, is neglect of care and constitutes abuse.
