Findings
The facility failed to provide a safe environment for the Alleged Victim by not having staff review the updated service plan until two weeks after updating it. The Alleged Victim had a fall resulting in a head laceration and bruising. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
