- On or about June 30, 2020, Alleged Victim (AV) received medication intended for another resident of the facility, resulting in AV being sent to the hospital for treatment. Alleged Perpetrator 2 (AP2) administered the medication error to AV which is considered neglect of care and constitutes abuse. An investigation determined that this is the second occurrence to happen to AV in approximately six months. The facility's failure to provide a safe medication administration system for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
