Findings
On or about June 27, 2020, Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) involuntarily secluded Alleged Victim (AV) by confining AV to his/her bed by using his/her wheelchair and furniture to prevent AV from getting out of bed. AP2's and AP3's involuntary secluded AV which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
