Findings
Alleged Victim (AV) had a known history of falls. AV's service plan was updated with interventions dated June 23, 2020. From June 23, 2020 through June 30, 2020, AV experienced six additional falls with only one new intervention. AV suffered injuries to his/her head, skin tears and re-opening wounds. The facility failed to plan care and implement appropriate interventions to mitigate AV's risk of injuries due to falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
