Findings
The facility failed to provide a safe environment and appropriate services to maintain the health and safety of the Alleged Victim's (AV). The failure resulted in AV's arm getting trapped between his/her arm and wheel chair for approximately 20 minutes, when no help arrived AV pulled his/her arm free causing skin injury and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
