Findings
Alleged Victim (AV) is prescribed a medication patch to be applied every seventy two (72) hours. On June 29, 2020, AV's patch was signed out by staff. On June 30, 2020, AV's patch was signed out by staff. On July 3, 2020, AV's patch was signed out by staff. On July 4, 2020, AV's patch was signed out by staff. AV's patch was applied early on both June 30th and July 4th, 2020. The facility failed to provide an adequate medication administration system creating a serious risk of harm to AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
