Findings
The facility failed to follow the Alleged Victim's (AV) care plan to monitor him/her while sitting in his/her chair due to a history of falls. The failure resulted in AV slipping from his/her chair to the ground, causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
