Findings
The facility failed to appropriately care plan and implement interventions that were effective regarding the Alleged Victim's (AV) falls to ensure safety. Between April and July, AV had approximately 10 different falls, some with and some without injury. AV fell on or about July 4, 2020 causing injury. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
