- Alleged Victim (AV) requires facility assistance with his/her medication management. On or about July 1st, 4th and 5th, 2020, AV did not receive all of his/her medication. AV did not suffer any adverse effects from missing his/her medication. The facility failed to administer a medication as ordered, which is a violation of Oregon Administrative Rules.
