Findings
According to documentation, Alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim's (AV) care plan according to his/her nutritional needs. AV asked AP2 for a breakfast item, which AP2 refused and did not offer AV an alternative or substitute. AP2's actions are considered neglect of care and constitutes abuse. The facility failed to protect AV from abuse, which is a violation of Oregon Administrative Rules.
