Findings
The facility failed to provide a safe environment for Alleged Victim (AV) by failing to protect AV from Witness 1's (W1) aggressive behaviors. An investigation determined that W1 was aggressive towards AV on or about July 15, 2020 and again on August 11, 2020. The facility failed to implement safety measures to protect AV which placed AV at risk of harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
