Findings
Alleged Victim (AV) requires staff assistance with activities of daily living. On or about July 12, 2020, Alleged Perpetrator 2 (AP2) was assisting AV with cares when AV grabbed at AP2 breaking his/her necklace. AP2 made derogatory comments to or in the presence of AV. AP2's actions are considered verbal/emotional abuse. The facility failed to protect AV from verbal/emotional abuse which is a violation of Oregon Administrative Rules.
