Findings
The facility failed to provide Alleged Victim (AV) with regular and consistent assistance with toileting and transfers which resulted in AV suffering from chronic wounds on his/her buttocks area due to AV's incontinence briefs not being changed in a timely manner. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
