Findings
The facility failed to provide a safe environment for Alleged Victim (AV) by failing to protect AV from Witness 1's (W1) aggressive behavior which resulted in AV being slapped by W1 leaving a handprint on AV’s arm. The facility's failure is a violation of resident rights, is considered neglect of care which constitutes abuse.
