The facility failed to provide a safe environment for Alleged Victim (AV) by failing to prevent a resident-to-resident altercation between AV and Witness 1 (W1) in which W1 shoved AV and exposed AV to serios risk of harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
