- An investigation determined that Alleged Perpetrator 2 (AP2) verbally assaulted and intimidated Alleged Victim (AV) which caused AV to have anguish, distress, fear, unreasonable emotional discomfort, loss of personal dignity, and/or loss of autonomy. AP2's actions is considered verbal abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
