Findings
On or about August 6, 2020, Witness #1 (W1) touched the Alleged Victim (AV) inappropriately. AV was caused unreasonable discomfort and was caused a loss of dignity due to W1's actions. The facility failed to protect AV from unwanted advances by W1, by not supervising AV in a social setting, as outlined in his/her care plan. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
