Findings
According to documentation, the Alleged Perpetrator 2 (AP2) used physical force to attempt to remove the Alleged Victim (AV) from a residents room. AV was aggressive and grabbed AP2's hair, AP2 then pushed AV causing him/her to experience a fall resulting in a fractured hip. Ap2's actions are considered physical abuse. The facility failed to protect AV from abuse, which is a violation of Oregon Administrative Rules.
