Findings
The facility failed to ensure supervision and staff support regarding known behaviors related to Witness #1 (W1). An incident occurred between W1 and the Alleged Victim (AV) where W1 pushed a bedroom door into AV, causing AV to fall and sustain injury. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
