Findings
The facility failed to provide a safe environment and supervision of the Alleged Victim (AV) according to his/her risk of falls. The failure resulted in AV experiencing an unwitnessed fall causing skin injury and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
