Findings
The facility failed to follow Witness 1's care plan to monitor and redirect him/her due to behaviors and prior altercations. The failure resulted in a physical altercation with the Alleged Victim (AV) causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
