Findings
Alleged Victim (AV) has a history of wandering and having prior elopements. On August 7, 2020, AV eloped from the facility through an unlocked perimeter gate and was later brought back to the facility by local law enforcement. Upon return to the facility, AV was observed to be exhausted, hungry, tired, and having a blister on his/her left foot. The facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
