Findings
On or about August 20, 2020, Alleged Perpetrator 2 (AP2) was sitting with Alleged Victim (AV) at the table for a meal. AV was pushing back from the table during the meal. AP2 forcefully grabbed AV's wrist to get him/her to remove them from the table. AP2's actions are considered physical abuse. The facility failed to protect AV from physical abuse which is a violation of Oregon Administrative Rules.
