Findings
On or about August 20, 2020, Alleged Perpetrator 2 (AP2) was supervising Alleged Victim (AV) during meal time. AV has a known history of wandering and needing redirecting during meal times. AP2 pushed AV's chair so AV could not push it out again and AP2 would not allow AV to get up from the table. AP2's actions are considered the use of a wrongful restraint and constitutes abuse. The facility failed to protect AV from a wrongful restraint which is a violation of Oregon Administrative Rules.
