Findings
On or about August 20, 2020, Alleged Perpetrator 2 (AP2) was helping Alleged Victim (AV) eat. AV has a known history of wandering during meal times and requires staff redirection. AP2 pushed AV's wheelchair up to the table so that he/she could not leave the table. AP2's actions are considered a wrongful use of a restraint and is abuse. The facility failed to protect AV from being restrained which is a violation of Oregon Administrative Rules.
