The facility failed to ensure supervision and staff support regarding known behaviors related to Witness #1 (W1). On or about August 14, 2020, W1 punched the Alleged Victim (AV), placing AV at risk for serious harm. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
